I just ran into this problem on my 99 Tahoe with a freshly rebuilt 4L60E Tranny. The problem was a long delay shifting from 2nd to 3rd sometimes even skipping right to overdrive. The problem was found. The previous owner installed an aftermarket performance enhancing box that connects right to the MAF.

The fluid was off bottom of dipstick so I dropped pan and changed filter, new fluid, hoping the low fluid was causing low pressure and perhaps as result, no overdrive. Now, -reverse fine. -shifts well from 1-2 and 2-3, though a little harder than previous test drive before fluid change, running at correct fluid level.

For reference the stock torque rating for the 4L60E is 360 ft/lbs and 380 ft/lbs for the 65E. I personally dont recommend more than a 50% reduction of TM on a stock trans.
